Targeted Case Management (TCM) offers services to adults in Baltimore County who lack basic support for income, shelter and food and are at risk of hospitalization, homelessness or incarceration. The service provides referrals and linkages to community resources in areas of medical, mental health, financial, legal, and other related areas. Visits are conducted in the community, home/shelter, or where requested by the participant to support wellness and recovery.

Through the process of assessment, planning, implementation, and monitoring, the Targeted Case Manager provides comprehensive and flexible services. Services are completed with the individual’s strengths, needs, abilities and preferences in mind. Emergency crisis intervention and conflict management services are also available when immediate intervention is necessary.

Targeted Case Managers advise, aid, and track in the process of engaging participants in gaining access to the full range of pertinent resources or services which may include:

  • Familial
  • Financial
  • Housing
  • Legal
  • Medical
  • Mental Health
  • Social
  • Transportation
  • Vocational
  • Others such as developing social support systems or community social integration
